NURS 5312 Advanced Management of Chronic Diseases II

3 SCH. This course will focus on evidence-based approaches for improving the care of diverse patient populations impacted by chronic health conditions while integrating a whole health approach to providing thorough discharge planning, patient education, and interdisciplinary care coordination.
