Applied Health Outcomes Research Certificate Admissions

Proficiency in English
Bachelor’s degree minimum GPA 2.75, OR
Professional students (example: PharmD students) without a conferred degree and overall GPA of 2.8 prior to their enrollment in professional degree program
Statistics or equivalent training (3 semester credit hours)
Justification may be submitted for candidates who do not meet all admissions requirements.

Application requires the following:

One-page personal statement of interest in the program (maximum 400 words)
Two letters of recommendation
Resume or CV
Submit official transcript(s) from all institutions attended. Transcripts must be official, sent directly to HSC from your institution(s). Official electronic transcripts may be sent to admissions@unthsc.edu.
A course-by-course WES International Credential Advantage Package (ICAP) is required from any international institution attended.
TOEFL Exam Scores are required for applicant who are not U.S. Citizens or Permanent Residents unless you have completed a degree program from within the US. TOEFL PharmGrad Code: 8246.
