Applied Health Outcomes Research Certificate Admissions

Academic Requirements

• Proficiency in English
• Bachelor’s degree minimum GPA 2.75, OR
• Professional students (example: PharmD students) without a conferred degree and overall GPA of 2.8 prior to their enrollment in professional degree program
• Statistics or equivalent training (3 semester credit hours)
• Justification may be submitted for candidates who do not meet all admissions requirements.

Application requires the following:

• One-page personal statement of interest in the program (maximum 400 words)
• Two letters of recommendation
• Resume or CV
• Submit official transcript(s) from all institutions attended. Transcripts must be official, sent directly to HSC from your institution(s). Official electronic transcripts may be sent to admissions@unthsc.edu.
• A course-by-course WES International Credential Advantage Package (ICAP) is required from any international institution attended.
• TOEFL Exam Scores are required for applicant who are not U.S. Citizens or Permanent Residents unless you have completed a degree program from within the US. TOEFL PharmGrad Code: 8246.
• Optional, published articles
• $50 application processing fee required
